Sequence Protocol

Fig. 1 - 3. On the conventional dynamic T1 gradient-echo MR angiogram the local resolution is limited [1]. Smaller muscle branches can only be vaguely distinguished (arrow). In the same patient, a high-resolution T1-3D gradient-echo sequence [2] in the steady-state phase about 7 minutes after injection of gadofosveset depicts very clearly not only the smaller muscle branches but also a small aneurysm (arrow), which is not detectable on the conventional images even with knowledge of the finding. The maximum intensity projection (MIP) of the high resolution sequence [3] shows the diffuse infiltrates of the myositis as high-signal cloud around the muscle vessels in the proximal third of the depicted area (arrow).

Diagnosis

Comparison of conventional contrast-enhanced versus high-resolution MRA in a patient with myositis. Visualization of a microaneurysm and perivascular infiltrates.
